Как отправить форму после модального диалога подтверждения jQuery?

Этот сценарий открывает диалоговое/модальное окно jQuery для запроса проверки перед отправкой формы, но когда пользователь нажимает кнопку «Отправить» в модальном окне, ничего не происходит. Firebug показывает, что проблема связана со строкой, содержащей "document.leaveReq.submit();" Я новичок в jQuery и javascript, и несколько дней боролся с этим. Любая помощь здесь очень ценится.
